First, you should go West Bengal's official web portal www.wbhealth.gov.com. then  comes to you log in to with your mobile number :

• enter your number to receive your number
• enter your OTP number and a new page opened
• enter  your hospital name
• enter Health ID number if you have yes or no
• Enter Visiting date
• Name
• Gender
• Age, month, day
• country
• State
• Police Station
• Full Address
• pin number
• OPD ( Select which purpose you visit the hospital)
• Doctor  name Auto Selected and Room Number
• Enter your health ID no and Health number
• Enter Adhaar Number ( not Mandatory )

After doing all these processes submit this page and a page will appear on your desktop or laptop, take a printout copy then visit your local hospital.
Also you get your Health report Online do not need to visit your hospital.
